Register, now, for the spring sports season
** NEW REGISTRATION PROCEDURES AND DOCUMENTS **
1. ONLINE REGISTRATION
A parent/guardian or 18 year old student must complete the new online registration form (Athletic Resource Fee Agreement) prior to trying out for the 20-21 spring season.
A separate form must be completed for each student.
2. PHYSICALS AND QUESTIONNAIRES
Please read the MHSAA Physicals and Questionnaire tab, below, carefully for changes and detailed instructions.
3. ATHLETIC RESOURCE FEE
Within 48 hours of your child officially making a team, the Athletic Resource Fee will be added to your child's MISTAR Student Body Account. You will then have five (5) calendar days to make your payment by one of the following methods:
Pay online through the MISTAR parent portal, SBA
A non-refundable convenience fee will be applied.
Hand a check or money order to the coach (payable to Berkley School District)
Write your child's name and sport on the check or money order to ensure your payment is processed correctly.
NOTE: All outstanding Athletic fees and team expenses from previous seasons/years must also be paid in full prior to participation.

- No more than one failing grade during the previous semester
- Have and maintain a GPA of 1.50 or higher from the previous semester
MHSAA Physicals and Questionnaires
** NEW **
Sports physicals that were submitted during the 2019-20 school year, dated on or after April 15, 2019, are currently on file with the Athletic department and are valid for the entire 2020-21 school year per the MHSAA.
All students who wish to participate during the 2020-21 school year, must submit ONE of the following documents to the Athletic office prior to tryouts/practice. Documents need to be faxed to 248-544-5860 or emailed to Mrs. Folk or Mr. Horn. (Due to COVID-19 and the need to social distance, we ask that forms not be brought to the Athletic office.)
1. FIRST-TIME BERKLEY ATHLETES
If your child is a new student in the Berkley School District or will be participating in a MHSAA sport for the first time, you must complete both pages (top to bottom) of the MHSAA Pre-Participation Physical and Medical History form and submit it as instructed above.
2. RETURNING ATHLETES (FIRST SPORT THIS YEAR)
If your child is participating for the first time this year and you submitted a MHSAA physical last year, you only need to complete both pages (top to bottom) of the 2020-21 MHSAA Sports Health Questionnaire and submit it as instructed above.
3. RETURNING ATHLETES (SECOND OR THIRD SPORT THIS YEAR)
If your child participated in a fall and/or winter sport this year, you only need to complete the Medical Treatment Consent and Emergency Information at the bottom of either form and submit this as instructed above. This is required every season.

Volunteering, Volunteer Driving & Chaperone Responsibilities
Parents are encourage to volunteer in classrooms, chaperone field trips, and be a volunteer driver. In order to volunteer, all parents volunteering in the classroom or during school sponsored functions or field trip must complete the Volunteer Release Form prior to any activities. Parents driving or chaperoning a field trip must have the Volunteer Driver Form and Chaperone Responsibility Form completed in addition to the Volunteer Release Form.
