Parents will now be able to pay Athletic Resource Fees online through the MISTAR parent portal.  A non-refundable convenience fee will be charged.  At this time, payments are being accepted for fall sports only.  Parents will receive an email with additional information.

Note:  In addition to paying the 2020-21 Athletic Resource Fee, all outstanding Athletic expenses from previous years must be paid in full.

You must also complete and submit the updated Athletic Resource Fee Agreement.  This can be emailed to or faxed to (248)544-5860.

If you choose not to pay online, hand in the Agreement and payment to your student's coach.

Eligibility Requirements

  • No more than one failing grade during the previous semester
  • ​​Have and maintain a GPA of 1.50 or higher from the previous semester

Sports Physicals and Registration Forms

One of the MHSAA forms listed below must be completed and turned in on the first day of tryouts/practice.  A Berkley staff member will be present at the tryout/practice locations to collect them.

Sports physicals that were submitted for the 2019-20 school year (one that was completed on or after April 19, 2019) are on file with the Athletic department and are valid for the entire 2020-21 school year.

    If you are a new student to Berkley or will be participating in MHSAA sports for the first time, you must submit a completed MHSAA Physical and Medical History form.

Volunteer Drivers

Volunteering, Volunteer Driving & Chaperone Responsibilities

Parents are encourage to volunteer in classrooms, chaperone field trips, and be a volunteer driver. In order to volunteer, all parents volunteering in the classroom or during school sponsored functions or field trip must complete the Volunteer Release Form prior to any activities.  Parents driving or chaperoning a field trip must have the Volunteer Driver Form and Chaperone Responsibility Form completed in addition to the Volunteer Release Form.
